Downloadable savings forms and information about the general conditions of our savings accounts

We have put together a helpful list of savings documents and what each form can be used for:

Use the Withdrawal / Transfer / Notice Request form to:

  • make a withdrawal from your savings account
  • transfer to another Family Building Society account
  • provide notice of account closure

If this is a passbook account, you should enclose your passbook with the withdrawal form.

Use the Change of address form form to let us know if you move home.

Use the Change of Name form to let us know if you'd like to change your name on your savings account. Once filled in and completed please post the form alongside an original or certified document as proof of your change of name to Family Building Society, Ebbisham House, 30 Church Street, Epsom, Surrey KT17 4NL.

Use the Lost or stolen passbook report form to let us know if your passbook has been lost or stolen.

Use the Direct Debit Mandate form to let us know if you're changing the bank or building society account from which you pay your subscription.

Please note that we'll need 10 working days following receipt for new instructions and five working days to change the subscription amount or collection date.

Use the Cash ISA Transfer Authority form to transfer your existing Cash ISA to the Family Building Society.

We will only accept full transfers in of current tax year subscriptions. We will not accept partial transfers of current tax year subscriptions however, partial transfers in of previous years' subscriptions are allowed.

Please note, transfers from Stocks & Shares ISAs aren't permitted.

Simply complete this form and send it to us along with a completed Cash ISA application form. We'll then notify your current ISA provider so that the transfer can begin in line with their terms and conditions.

Please note this may take up to 15 business days, in addition to any notice period that may be required and they may make a charge for the transfer.

Use the Cash Junior ISA Transfer Authority form to transfer your existing Cash Junior ISA to the Family Building Society.

Please note, transfers from Stocks & Shares ISAs aren't permitted.

Simply complete this form and send it to us along with a completed Cash Junior ISA application form. We'll then notify your current ISA provider so that the transfer can begin in line with their terms and conditions.

Please note this may take up to 15 business days, in addition to any notice period that may be required and they may make a charge for the transfer.

Use the Power of Attorney form  to let us know you hold a Power of Attorney for one of our members.

Please note you'll also need to return to us an original or certified copy of the Power of Attorney documents and identification for each Attorney.

Use the deputy details form to register a Court of Protection Order.

Please note you'll also need to return to us an original copy of the Court of Protection Order and identification for each Deputy.

Use the third party mandate form  to tell us we can accept instructions regarding your account from a specific named person of your choosing ('the Third Party').

Use the Supplementary Application form to let us know if the Savings Account is to be operated by a trustee or trustees on behalf of a beneficiary.

If you need to change your nominated bank account you will be required to provide us with a bank or building society statement for that account dated within the last three months which shows your full name and address.

Complete a ‘Authority to Release Information’ form to authorise us to provide information or documents regarding your account(s) to a specific named person of your choosing.

 This agreement is for 12 months, after which a new signed authority form would be required to renew the arrangement.
